Job Overview We are seeking a dedicated and compassionate
Registered Nurse Case Manager to join our healthcare team. The
ideal candidate will play a crucial role in coordinating patient
care, ensuring that patients receive the appropriate medical
services and support throughout their healthcare journey. This
position requires a strong background in patient assessment and
care management, particularly for patients with complex medical
needs, including geriatrics and behavioral health. Responsibilities
Conduct comprehensive patient assessments to develop individualized
care plans. Coordinate primary care services and ensure continuity
of care across various healthcare settings. Maintain accurate
medical records and documentation for all patient interactions.
Perform diagnostic evaluations to identify patient needs and
recommend appropriate interventions. Engage in utilization
management to optimize resource allocation and improve patient
outcomes. Collaborate with interdisciplinary teams to facilitate
effective communication and care delivery. Provide education and
support to patients and their families regarding treatment options
and health management strategies. Monitor patient progress and
adjust care plans as necessary, particularly for those in ICU or
requiring specialized attention. Experience Current Registered
Nurse (RN) license in good standing. Proven experience in patient
care, particularly within primary care settings or case management
roles. Familiarity with medical records management and
documentation practices. Experience working with geriatric
populations and understanding their unique healthcare needs.
Knowledge of behavioral health issues and the ability to conduct
appropriate assessments. Strong skills in diagnostic evaluation,
critical thinking, and problem-solving. Previous experience in
utilization management is preferred but not required. Shift: Day
5x8-Hour (08:00 - 16:30) Join us in making a difference in the
